New Patient Initial Chiropractic Evaluation

New Patient Initial Evaluation - $175
This initial appointment is for new patients and includes: paperwork, consultation, evaluation and possibly chiropractic adjustment. This appointment is approximately 40 minutes. It is recommended you arrive 10 minutes early. All session times are approximate and include: setup, initial patient paperwork, payment, consultation, evaluation, chiropractic adjustment (if time permits) and cleanup. In special cases no chiropractic adjustment may be provided on this initial visit, this will be determined by the doctor.
Arriving late for your scheduled appointment will shorten the time with the doctor, with full charges being applied.
Please be aware that cancellations require 24-hours advanced notice. Late cancellations with less than 24 hours notice or "no shows" will be charged at the full rate of the scheduled appointment.
If you have insurance a receipt with diagnosis and procedure codes will be provided for you to submit to your insurance provider for direct reimbursement. The office does not bill insurance and is not a contracted HMO provider.
As with any primary care doctor’s office unavoidable emergencies sometimes occur. Your cooperation and understanding is appreciated.
Fees to be paid at time of service: Cash, Personal Check, Venmo, CashApp, Visa, Master Card, Discover Card, American Express, PayPal.
***Payments made by credit card/debit card/PayPal will be charged an additional 3% fee.

Chiropractic Adjustment

Chiropractic Adjustment - $55
This appointment only for existing patients in the office, only after an initial new patient evaluation has been completed. All session times are approximate and also include in addition to your care; setup, payment, treatment, cleanup.
